The surgery telephone lines are open between 8.30-11.00am  &  2.00-4.00 each day [Mon - Thurs],   and 8.30-11.00am on a Friday.   Please telephone during these times.   Thank you.

Prescription Access Request Form

If you would like to register to enable you to order your repeat prescriptions on-line, please request a patient on-line registration form from reception or print the form below. 

Your completed form should be passed to reception.   Once completed we will arrange for you to be given secure access [login/password] to order repeat prescriptions online.
